Outcomes

An outcomes story describes how well the program met its goal including long-term outcomes like changes in health indicators, organizations, and systems.
Process and Implementation
(2 pages)
Process and implementation stories detail how well the program is working and include short-term and intermediate outcomes like process changes, partnership creation, knowledge, attitude, or skill changes.
Lessons Learned
(2 pages)
A lessons learned story conveys challenges, approaches, and insights learned along the way that could help other communities learn and grow their prevention efforts.
